Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment. By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

Corona Virus

Late Patients
If you are late for your appointment, the reception staff will check with the GP to see if you will still be seen, however if you are more than 10 minutes late you will have to re-book. If you feel you must be seen, please speak to the receptionist who can update the GP.

The practice accepts patients new to the area and without a GP.    

All patients are required to show proof of identify by way of passport  (non-passport holders  other photo id) and proof of residence eg a utility bill showing current address.   Other proof may be requested depending on circumstances.

 

Registration paperwork will only be handed out once ID and proof of address has been seen. 

 **IN THE CURRENT CIRCUMSTANCES, PLEASE SEND AN EMAIL TO greatwestern.administrator@nhs.net TO REQUEST A FORM BE EMAILED OUT TO YOU - PLEASE SEND IT BACK WITH A COPY OF ID IN ORDER FOR US TO COMPLETE YOUR REGISTRATION**
